Little Dreamers Guide for Parents

Ok, so you've finished the Needs Assessment and the Waitlist Application--now what?

​

You are in a waiting period until you receive email confirmation that your family has been selected for a childcare slot to continue with Enrollment & Registration. 

​

Here are some things you will need to think about and gather for Enrollment & Registration

1) Knowing your work schedule to determine your needed childcare hours

​

2) Family History & Information

This form is how we as a daycare first establish a partnership with you. It will include:

-full legal names + nicknames

-Occupation/Employer

-Child's primary Residence

-Primary Language

-Family History (how many total children in family)

-Who all lives at Primary Residence with child

-If there's been change in family dynamic (divorce, separation, death, newborn, birth of another child)

-Religion background

-Cultural background

-Discipline style 

-Various questions about your child's interests, previous childcare experiences if any,

-additional information about child's learning style, what helps them process emotions

-Allergies, strong dislikes, phobias, triggers

-Parent Involvement: time you would like to volunteer at the Center or volunteer at Fundraisers

Child's Health History and Emergency Care Plan

This form needs to be completed before the child's first attendance date. 

-name of Physician and name of medical facility

-Sunscreen & Insect Repellent Authorization

-Health History: any special medical condition (asthma, diabetes, epilepsy, food allergies, non-food allergies, etc...)

-triggers that may cause problems

-signs or symptoms to watch for

-detailed steps We should follow, especially if prescription medication is necessary

 

Child Health Report-Date of child's most recent blood lead test

If your child is UNDER 2 yrs of age

-Health Examination must be completed within 6 mos of child's first attendance date OR no later than 3 mos after the child's first attendance date.

-Follow-Up Examination must be completed every 6 mos

 

If your child is age 2-5 yrs

Health Examination must be completed within 1 year of child's first attendance date  OR no later than 3 months after the child's first attendance date

-Signed authorization by Health Professional stating the child is able to participate in child care activities

​

Child's Immunization Record within 30 days of child's first day of attendance

​

​

This article is just an introduction. You will also need to fill out additional forms. That information will come out as we get closer to Enrollment.

Here is an EXAMPLE of Enrollment packet. You can use this as a guide to look & plan ahead for Enrollment. Please do not fill these out, we haven't started Enrollment yet.
